About this template
Before a central system is replaced: the existing equipment as on its nameplates, the static pressure against the rating plate, the line sets against the new unit's manual, the venting as found, and the licensed trades.
Guide and sources
Before a furnace, a central air conditioner or a central heat pump is replaced, record what is there. Existing equipment: one row per unit, the nameplate as printed (type, brand, model, serial, year, capacity, refrigerant, electrical, efficiency) and how it comes out. The capacity is a record: the replacement is sized from a load calculation (CSA F280-12 in Canada, ACCA Manual J in the US), never from this sheet.
Ducts and static pressure: the total external static measured with your manometer and the maximum from the rating plate (Goodman, Trane); the sheet shows the margin. Line sets: the length and the vertical separation as measured, the tube sizes, your plan, and the new unit's maximums from its manual (Rheem RP17AY: 150 ft, 50 ft of separation; an R-410A or R-22 line set purged and flushed with nitrogen, replacing it recommended). Venting: each appliance on a vent, the vent as found and whether another appliance shares it (Trane: a removed Category I furnace can leave the vent wrongly sized for the appliances still attached).
A survey, never a design: nothing is sized, no load is computed and nothing is judged; the margins only show the distance to a maximum typed from a manual. The electrical, gas and refrigerant work belongs to licensed trades: in Quebec, an RBQ licence with subcategory 16 for electrical work (the CMEQ qualifies these contractors) and the appropriate RBQ licence for gas work (15.1 for forced-air heating), and a certificate H to handle refrigerant; in the US, EPA Section 608 for refrigerant, and check what your state or city requires. The client's report shows the equipment and the measurements; the model and serial numbers, the manual maximums and the margins stay in your team's report.
